The following are the forms that need to be returned to me on or before August 23rd. There are 3 different forms that need to be returned. Some do not apply to every student.
Welcome Letter 1 (Orchestra Syllabus) The first form is the syllabus agreement and information sheet. This is the last page of the syllabus. It needs to be torn off the back, filled out, and turned in. This information should be the most current available. I like to send out information by email, so please provide an email address that is current and checked often. Please print clearly. The rest of the syllabus is for you to keep. 2 (Trip Permission Form) The top is already filled out. You need to fill out the student information section, the emergency contact section, and sign at the bottom. We do use this form at the beginning of the year as a blanket for the whole year. This form allows your child to ride the bus to and from events and allows medical attention to be given in case of an emergency. The school requires this form, so your child will not be allowed to perform at any event until this form is completed and turned in. Please note that the emergency contact on this form should be someone other than the parent or guardian. 3 (Instrument Contract) The contract will be given to the students when they check out an instrument. If the student is using a school instrument this form must be filled out. Although most students take good care of their instruments, this form is to protect the music program and its instruments.
