The following are the forms that need to be returned to me on or before August 4th. There are 9 different forms that need to be returned. Some do not apply to every student.
Welcome Letter 1 (Marching Band Syllabus) and 2 (Band Advanced Syllabus) Both of these are for all band students only. The last page of the syllabus needs to be filled out and turned in. The rest of the syllabus is for you to keep. This information should be the most current available. I like to send out information by email, so please provide an email address that is current and checked often. Please print clearly or type into the form. 3 (Colorguard Syllabus) and 4 (Dance Exploration Syllabus) Both of these are for all Colorguard students only. The last page of the syllabus needs to be filled out and turned in. The rest of the syllabus is for you to keep. This information should be the most current available. Please print clearly or type into the form. 5 (Trip Permission Form) The top is already filled out. You need to fill out the student information section, the emergency contact section, and sign at the bottom. We do use this form at the beginning of the year as a blanket for the whole year. This form allows your child to ride the bus to and from events and allows medical attention to be given in case of an emergency. The school requires this form, so your child will not be allowed to perform at any event until this form is completed and turned in. Please note that the emergency contact on this form should be someone other than the parent or guardian. 6 (Band Supply Form) or 7 (Colorguard Supply Form) The next form that needs to be turned in is the supply order form. There is one specific for band and one for guard. Each new student will be given one t-shirt free. If you would like to order more you may. Please indicate the size you would like. Each additional shirt is $15. You may buy as many shirts as you would like. All returning students can use their t-shirt from last year. If you would like a new one, it is $15. If you need a new pair of shoes please indicate the size. We have samples of the shoes at school if you want to try some on. Returning students, please make sure your shoes are in good condition and still fit. This form needs to be turned in even if you do not need anything. 8 (Instrument/Equipment Contract) The contract will be given to the students when they check out an instrument. If the student is using a school instrument/equipment, including percussion and guard, this form must be filled out. Although most students take good care of their instruments, this form is to protect the music program and its instruments. Below is a list of each instrument that your students can check out. Each instrument contract is different. Flute - Clarinet - Bass Clarinet - Alto Sax - Tenor Sax - Baritone Sax Trumpet - Horn - Trombone - Baritone - Sousaphone - Tuba - Percussion - Guard 9 (Band Uniform Contract) or 10 (Guard Uniform Contract) This form must be filled out and signed by all band and guard students. Same as the instrument contract, this is to help protect our uniforms. (Adult Volunteer Information Sheet) It takes a lot of work to make a marching band run smoothly. We can always use all the help we can get. Please sign-up even if you do not know how to do any of these jobs. We always have veteran parents who are willing to help you.
